My site has existing members. Using DNN 5.4.4, full trust environment, and set to a single worker process. I have used the advance login, but now upgraded to the new advancedlogin 45.2.01 and added in the facebook connected.
I setup the facebook services in facebook, the system is allowing a login and then the "You have been successfully authenticated, however this authenticated identity is not a member of the site." So I associated with the existing account. I then logged off, and reconnected with the facebook login. I then get this same screen and when I associate with existing account the error "Cannot insert duplicate key row in object dbo.userauthentication with unique index.

We have logged in and taken a look. What we found is that the Facebook account was associated twice with the DNN account. This was the cause of the problem.
We removed both instances. - Then tested with another account.
We then removed that, so you are clean and ready to recreate the account association with facebook. Ensure you do it only 1 time.
