Installation Issues on Licensed Control

I just installed the latest version of the Advanced Login module to my website and when I go into the "Configure Module" page, the page contents are blank and upon closer examination the javascript console reports 6 exceptions.
Uncaught TypeError: Object function getElementById() { [native code] } has no method 'addEventListener'

I urgently need assistance on this. The website is running DotNetNuke 4.9.4 on IIS7. The SKIN is the default Minimal Extrophy skin on the edit page.
The User Name and password fields appear on the control when I am not logged in, and it is functional, but as soon as I login as HOST, I have nothing on the control and cannot change any settings on it.
