We had to change our DB password post-installation and now Complete Feedback Designer isn't playing nice. Other modules and the rest of the site is working fine, so it just seems to be an issue with Complete Feedback Designer at this point.
Perhaps someone can shed some light on this issue?
I've already tried uninstalling/reinstalling feedback designer, but I still get this error:

Are you still having trouble with this? check it was only a password change (and not username change also, the sql created tables may be under owner of previous username).
If all else fails, uninstall the module, check the database, be sure there are no lingering stored procs or tables. (double check all db account references in your web.config file) Reinstall the module.
