I'm getting the red X where one of those icons should be showing up. The Send/Reset buttons show up fine. When I viewed source, here's what I noticed:

The SEND button that works
src="/DesktopModules/Complete&20Feedback&20Designer/Images/send7.gif"

The REQ.GIF and NONREQ.GIF icons that DON'T work
src="DesktopModules/Complete&20Feedback&20Designer/Images/...

Notice the lack of / on the req.gif and nonreq.gif images? This is obviously causing the problem, but I can't figure out how to fix it.

Any suggestions?

Can we grab some additional data from you.
 
1. A url where we can see the form?
2. The module version you are using?
3. The basica process you used to set or upload the image?
4. The DNN version.
