Using DNN version 04.03.05
Import of module showed no errors but when I add it to a page I get:

Error: Complete Feedback Designer is currently unavailable.
DotNetNuke.Services.Exceptions.ModuleLoadException: An attempt to attach an auto-named database for file <database path and name>.mdf failed. A database with the same name exists, or specified file cannot be opened, or it is located on UNC share.

Can you check if you have a module called - Feedback Designer, and not Complete Feedback designer.
 
Add the module called "Feedback Designer" This replaced the earlier version of the product.
 
Let us know how you go.
